Sign up for Sudden Cardiac Arrest (SCA) screening
Dear Parent/Guardian:
The Lindenhurst School District has received a grant from the Heart Screen New York Saving Lives through Early Detection Program (Heart Screen New York). This is a unique opportunity for your child to have a Heart Health Survey, a Heart Sounds Physical, (ECG) Electrocardiogram and Echocardiogram, at no cost or obligation to you! The importance of cardiac screenings and early detection in youth is most important! Please complete the following enclosed forms if you choose to have your child participate in this screening, to be held on Saturday, September 28, 2024.
• Agreement to Participate in Heart Screening (2 pages) signed by a parent or guardian.
• Heart Health Questionnaire (5 pages) please fill out with your child, their input is valuable.
• Duke University Study (1 page) optional.

1. What can my child expect?
Arrive at assigned appointment time with heart history survey completed and consent form signed by parent. All testing will occur in the HS gym with separate screening areas for male and female students, and will be done in a group type setting. (Screening may be done in a private screening room if requested).
Males - Change into paper gown (shirts off, pants stay on)
Females - Change into paper gown (shirts off, please wear a sports bra, pants stay on)
Blood Pressure
ECG/EKG
Height/Weight and Heart Sounds Physical
Education Table (CPR/AED demonstration)
Consult with Cardiologist
Echocardiogram if needed, back to Cardiologist consultation
2. The entire testing session usually takes approximately 45 minutes, unless an Echo is needed.
3. Your child cannot have any type of physical exercise 1 hour prior to screening. Your child
cannot us any creams or lotions on day of screening
To assure privacy, please email completed forms to athletics@lufsd.org or drop forms off at the McKenna Administration in a sealed envelope with your child’s name on the outside.
The screenings will be held on Saturday, September 28th from 8:30 am to 3:00 pm in the Lindenhurst High School Gymnasium. (Screening forms must be received prior to September 13th).
Once all paperwork is submitted, families will be contacted with an appointment time. All participants will receive a summary of the test results from a cardiologist; and if further follow-up testing is recommended, you will be notified at the screening so you will have the opportunity to speak with the physician. We encourage parents to accompany their child during the testing if possible.
