The Lindenhurst Union Free School District recognizes that good student health is vital to successful learning and acknowledges its responsibility, along with that of parent(s) or guardian(s), to protect and foster a safe and healthful environment for the students.
If your child’s health status changes, please notify his/her school nurse so that we may accommodate any health needs that your child may have. If your child is having surgery, or is diagnosed with any health condition, please contact the nurse’s office at your earliest convenience.

Student Health Examinations
Annual health examinations are required for all new student entrants as well as for students in Grades Pre-K or K, 1, 3, 5, 7, 9, and 11, students participating in sports, students who need working permits and needed triennially for the Committee on Special Education (CSE). The ‘NYS School Health Exam Form’ needs to be completed by the parent/guardian and physician. This form can be found under the Medical Forms & Documents section below.
Student Immunization Requirements
New York State (NYS) Public Health Law Section 2164 and New York Codes, Rules and Regulations (NYCRR) Title 10, Subpart 66-1 require every student entering or attending public, private or parochial school in New York State (NYS) to be immune to diphtheria, tetanus, pertussis, measles, mumps, rubella, poliomyelitis, hepatitis B, varicella, haemophilus influenzae type b, pneumococcal and meningococcal in accordance with Advisory Committee on Immunization Practices (ACIP) recommendations.
In addition, for the 2020-21 school year, the meningococcal immunization is required for grades 7, 8, 9, 10, 11 and 12. Visit the New York Department of Health website for school vaccination requirements.

Administration of Medication to Students
If your child will be taking medicine at school, provider and parent permission forms are needed for both prescription and over the counter medications. New medication permission forms are required each school year. New York State Law requires that:
We request that you ask your pharmacist to give you a second identically labeled container for any prescription medications your student will take at school. We also request that you bring small containers of any OTC medications that your student will take. This will allow the School Nurse to send these medications on field trips and comply with New York State laws pertaining to medication storage. Medication forms may be obtained below under Medical forms & Documents or directly from the School Health Office.
Dental Health
In addition to a physical examination, students will be asked to provide a dental health certificate when they enroll in school in accordance with the same schedule as the health certificate. The Dental Health Certificate can be found under "forms".
Weight Status Category (BMI Opt-Out)
The State of New York may authorize and require the collection of height and weight data from health certificates in order to track and understand health care issues that affect children. Lindenhurst School District will report information about our student's weight status groups, without sending individual names or information, when requested by the New York State Department of Health. However, parents may choose to have their children's data excluded from the reporting of this information. Parents who choose to opt-out of reporting may complete the Weight Status Category (BMI Opt-Out) form (under "forms) and return it to the Health Office in their school.
